Application for Change of Department for Enrolled Students, Fall 2026 This is to notify the application for change of department/major for enrolled students for Fall semester, 2026. Those who wish to change departments or majors are required to submit the application form within the designated time as below. 1Eligibility a.Any of the degree courses (Master's, Ph.D. and Integrated Courses) b.Enrolled students who completed first or second semesters 2Required Documents a. Applying for 'Change of Department' i)Application form for change of department (Prescribed form): Attachment #1 ii)Application form for Credit Recognition from the change of department: Attachment #2 iii)Transcript b.
